Briviact (brivaracetam) is a twice-daily oral antiepileptic — a more selective successor to levetiracetam (Keppra) targeting synaptic vesicle protein 2A (SV2A). It is used for partial-onset seizures, often as add-on therapy in patients inadequately controlled on other AEDs.

How Briviact Works:

Partial-onset seizures arise from a focal area of the brain. SV2A is a synaptic vesicle protein involved in neurotransmitter release. Modulating SV2A reduces excessive neurotransmitter release contributing to seizures.

Brivaracetam binds SV2A with ~20x higher affinity than levetiracetam. The clinical advantage over levetiracetam may include faster onset and potentially better tolerability regarding behavioral side effects (irritability, depression) that affect some patients on Keppra — but evidence for clear superiority is modest. Generic levetiracetam works well for most patients at a fraction of the cost.

Form and use:

Briviact is taken by mouth twice daily, with or without food. Tablets, oral solution, and IV form available. Starting dose 50 mg BID; max 100 mg BID. Renal/hepatic dose adjustment may be needed.

Generic availability:

Generic brivaracetam is now available. Generic levetiracetam (Keppra IR and XR) is widely available at very low cost. Other AEDs for partial-onset seizures: lamotrigine, oxcarbazepine, lacosamide (Vimpat, generic), perampanel (Fycompa), carbamazepine, valproate, and many others.
