Arixtra (fondaparinux) is synthetic pentasaccharide selective factor Xa inhibitor — derived structurally from heparin’s antithrombin-binding sequence. Indirect anticoagulant: binds antithrombin, dramatically accelerating Xa inhibition. No effect on thrombin. Less risk of heparin-induced thrombocytopenia than heparin or LMWH.

How Arixtra Works:

Fondaparinux binds antithrombin’s heparin-binding site, conformational change increases ATIII inhibition of factor Xa by ~300x. No direct thrombin inhibition.

Once-daily SC dosing for both prophylaxis (2.5 mg) and treatment (5–10 mg depending on weight). Renal clearance — adjust for CrCl <50; avoid CrCl <30.

Form and use:

Arixtra SC injection daily.

Generic fondaparinux available. Generic enoxaparin (LMWH alternative) very cheap. DOACs (Eliquis, Xarelto) — convenient oral alternatives for many indications, all brand currently.
