It combines rilpivirine, which binds to a non-active site to disable the enzyme, with emtricitabine and tenofovir alafenamide (TAF), which terminate the growth of the viral DNA chain. The use of TAF allows for high intracellular effectiveness with lower plasma levels, resulting in reduced toxicity to the kidneys and bones.
Form and use:
This medication is a once-daily oral tablet that must be taken with a meal containing at least 533 calories to ensure proper absorption of the rilpivirine component. Failing to eat a full meal or taking the pill only with low-calorie liquids can lead to reduced effectiveness and potential treatment failure. Additionally, patients using acid-reducing medications must follow specific timing precautions to avoid drug interactions.
Generic availability:
As of 2026, there is no generic version of Odefsey available in the United States. While other rilpivirine-based options like Complera and the injectable Cabenuva exist, current medical guidelines often favor integrase-inhibitor-based regimens for patients starting new treatments. Rilpivirine absorption depends on stomach acidity. Proton pump inhibitors (omeprazole, esomeprazole, etc.) are contraindicated. H2 blockers (famotidine, ranitidine) must be taken at least 12 hours before or 4 hours after Odefsey. Antacids (Tums, Maalox) must be taken at least 2 hours before or 4 hours after Odefsey.
