Mepron (atovaquone) is an oral antiprotozoal used for the prevention and treatment of mild-to-moderate Pneumocystis jirovecii pneumonia (PJP) in immunocompromised patients who cannot tolerate trimethoprim-sulfamethoxazole.
How Mepron Works:
Atovaquone targets Pneumocystis jirovecii, an opportunistic fungus that can cause fatal pneumonia in immunocompromised individuals. It works by inhibiting the cytochrome bc1 complex, which interferes with mitochondrial electron transport and blocks the parasite’s energy production. This fungistatic action also extends to other parasites like Toxoplasma and Plasmodium by disrupting their pyrimidine synthesis.
Form and use:
Mepron is provided as a yellow oral suspension and must be taken with food, specifically high-fat meals, to ensure proper absorption and effectiveness. For PJP prevention, the standard dose is 1500 mg once daily, while mild-to-moderate treatment requires 750 mg twice daily for 21 days. Taking this medication on an empty stomach significantly reduces its clinical performance.
Generic availability:
While generic atovaquone is available at a lower cost than brand-name Mepron, it is typically reserved for patients who cannot tolerate first-line treatments like trimethoprim-sulfamethoxazole (TMP-SMX). TMP-SMX remains the primary choice for prevention due to its affordability, often costing between $4 and $15 per month. Other generic alternatives for PJP prevention include dapsone and aerosolized pentamidine.
Warnings:
Mepron is not recommended for severe PJP cases, which instead require inpatient treatment with IV pentamidine or dose-adjusted TMP-SMX. Patients should be aware of potential hepatotoxicity, hypersensitivity reactions, and drug interactions with medications like rifampin, tetracycline, or metoclopramide that reduce atovaquone levels. Additionally, gastrointestinal issues such as diarrhea can interfere with the drug’s absorption.

Atovaquone is poorly absorbed without food. Taking with a meal — especially one containing fat — substantially increases absorption (about 3-fold higher levels with high-fat food vs. fasting). Skipping food can reduce drug levels enough to fail prevention or treatment.
Yes. Generic atovaquone oral suspension is FDA-approved and clinically equivalent to brand Mepron. Many patients can access generic at lower cost.
Yes. Rifampin and rifabutin substantially reduce atovaquone levels (rifampin is contraindicated). Tetracycline reduces atovaquone levels. Metoclopramide reduces atovaquone levels. If you take any of these medications, tell your prescriber. Atovaquone may slightly raise zidovudine levels and slightly reduce indinavir levels.
For prevention, typically as long as you remain immunocompromised (HIV with low CD4, on immunosuppression, etc.). For treatment, typically 21 days. Your provider will guide duration based on your underlying condition.
