Simponi is a fully human monoclonal antibody that targets TNF-alpha, an inflammatory cytokine responsible for joint destruction and intestinal inflammation in conditions like RA and UC. Form and use:
This medication is typically administered as a 50 mg subcutaneous injection once a month using a prefilled syringe or an autoinjector. For patients with ulcerative colitis, a higher induction dose is required at the start of therapy before moving to the monthly maintenance schedule. While an IV version exists with different requirements, the subcutaneous form can be self-administered at home after proper training.
Generic availability:
As of 2026, there is no FDA-approved biosimilar version of Simponi available in the United States. While several other TNF inhibitors, such as Humira and Remicade, have lower-cost biosimilars on the market, Simponi remains available only as a brand-name product. Choosing between Simponi and these alternatives depends on a patient’s specific diagnosis, medical history, and overall tolerability.
Warnings:
Simponi carries serious warnings regarding life-threatening infections, including tuberculosis (TB) reactivation and sepsis, as well as risks for certain malignancies like lymphoma. Patients must undergo TB screening before beginning treatment and should be monitored for heart failure, hepatitis B reactivation, and demyelinating diseases. Any new symptoms, such as persistent cough, fever, or unusual fatigue, should be reported to a doctor immediately.
