Orencia (abatacept) is a biologic medicine used to treat several inflammatory joint conditions, including moderate-to-severe rheumatoid arthritis in adults, active psoriatic arthritis in adults and children 2 and older, and active polyarticular juvenile idiopathic arthritis in patients 2 and older. It is available either as an intravenous infusion or as a once-weekly subcutaneous injection.
How It Works:
Rheumatoid arthritis, psoriatic arthritis, and polyarticular juvenile idiopathic arthritis are all driven by an overactivated immune system that attacks the joints, causing inflammation, pain, swelling, and joint damage over time. T cells are central to that overactivation, and they need a specific ‘second signal’ (called co-stimulation) from antigen-presenting cells to become fully active. The co-stimulation signal travels through CD80 and CD86 molecules on antigen-presenting cells, which connect to CD28 on T cells.
Orencia is a fusion protein that binds to CD80 and CD86 and physically blocks them. With CD80 and CD86 occupied by Orencia, antigen-presenting cells cannot deliver the co-stimulation signal to T cells, so T cells do not become fully activated against the joints. The result is reduced joint inflammation and slower joint damage.
Form and Use:
The subcutaneous form is available as a 125 mg prefilled syringe or auto-injector (ClickJect pen) and is typically given once a week. The intravenous form is given as a 30-minute infusion at Weeks 0, 2, and 4, and then every four weeks thereafter, with the dose based on body weight. Orencia is generally not used together with TNF antagonists, other biologic DMARDs, or JAK inhibitors.
