Truqap (capivasertib) is a prescription oral tablet used in combination with fulvestrant to treat adults with hormone receptor-positive (HR+), HER2-negative locally advanced or metastatic breast cancer that has one or more PIK3CA, AKT1, or PTEN gene alterations, and that has progressed on or after endocrine-based therapy. It is the first and only FDA-approved AKT inhibitor. Available in 160 mg and 200 mg tablets, taken as 400 mg twice daily on a 4-days-on, 3-days-off schedule.
How It Works:
In many HR-positive breast cancers that have become resistant to hormone therapy, the cancer cells have found a way to keep growing by activating a signaling pathway called the PI3K/AKT pathway. This pathway acts like an accelerator pedal for cell growth — and in approximately half of resistant breast cancers, one of three genes (PIK3CA, AKT1, or PTEN) has mutated or been altered, causing the AKT protein to become permanently overactive.
Truqap is the first medication designed to directly block the AKT protein itself. When you take Truqap, the capivasertib molecule enters the cancer cell and binds to all three versions of the AKT enzyme (AKT1, AKT2, and AKT3), preventing them from sending growth signals. With AKT blocked, the cancer cells lose the survival advantage that made them resistant to hormone therapy. Truqap is taken in combination with fulvestrant, creating a two-pronged attack against estrogen-driven cancer growth.

Available in 160 mg and 200 mg tablets. Take 400 mg (two tablets) twice daily for 4 consecutive days, then take no Truqap for 3 days. This 4-days-on, 3-days-off cycle repeats continuously.
