Otezla (apremilast) is a twice-daily prescription oral tablet used to treat adults with plaque psoriasis who are candidates for phototherapy or systemic therapy, adults and children (age 6 and older) with active psoriatic arthritis, and adults with oral ulcers associated with Behçet’s disease. An extended-release formulation (Otezla XR) is also available for once-daily dosing in certain patients. Unlike many psoriasis and psoriatic arthritis treatments that require injections, Otezla is taken by mouth — offering a convenient option you can take at home. Treatment begins with a 5-day dose titration to help your body adjust.

How it works:

Psoriasis, psoriatic arthritis, and Behçet’s disease are all driven by an overactive immune system that produces excessive inflammation. Inside your immune cells, an enzyme called phosphodiesterase 4 (PDE4) plays a key role in regulating inflammation. When PDE4 is overly active, it drives the production of inflammatory molecules (cytokines) that cause the immune system to attack your own skin, joints, and tissues — producing plaques, joint pain, and oral ulcers.


Otezla works by blocking PDE4 inside immune cells. When PDE4 is inhibited, the balance of inflammatory signals shifts — the production of pro-inflammatory cytokines decreases, while anti-inflammatory cytokines increase. This rebalancing calms the overactive immune response without broadly suppressing your entire immune system the way some other autoimmune treatments do. For psoriasis, this means less skin cell overgrowth and clearer skin. For psoriatic arthritis, less joint inflammation and reduced pain. For Behçet’s disease, fewer and less severe oral ulcers.
